The Community Health Improvement Plan (CHIP) was created for a long-term, systematic effort to address public health problems on the basis of the results of the community health assessment activities and the implementation strategies chosen to address community needs. The purpose of the CHIP is to describe how a host of community partners will work together to improve the health of the region our collective public health departments serve. The planning and implementation process is community driven. This plan is used, in collaboration with community partners, to set priorities and align activities in the Tri-County area.
This plan will be presented at a community forum event in January 2017 to community partners, agencies and residents in the Tri-County Region.
