COVID Vaccine Information

COVID Vaccines are available by appointment. Please contact our office to schedule.

  • COVID vaccines are available to most adults living in the US at no cost through their private health insurance, Medicare, and Medicaid plans. Check to make sure that the location you choose to administer your vaccine is "in-network" with your insurance plan.
  • Adults 18 years and older without health insurance and adults whose health insurance does not cover all COVID-19 vaccines can get updated COVID-19 vaccines at no cost through the Bridge Access Program. Visit vaccines.gov to find providers that offer no-cost COVID-19 vaccines through the Bridge Access Program. 
  • Visit Vaccines.gov to find additional locations to receive your vaccine

Lost your Covid Vaccine card? 

  • IDPH Vax Verify (link) 
    • For issues using Vax Verify, call 800-889-3931
  • Check your MyChart for records (if you do not have MyChart account, check with your physician to sign up)

Please note that the Health Department cannot replace lost/missing vaccine cards.
