The Woodford County Board of Health is seeking nominations for the 11th Annual Public Health Award. The award is given annually in celebration of National Public Health Week. Nominees should be a resident of Woodford County who has dedicated their time to help improve the health and well-being of the community. If you would like to nominate someone for this award, please complete a nomination form. The form can be found on the Woodford County Health Department website, www.woodfordhealth.org. Submissions can be emailed to the Woodford County Health Department Administrator, Hillary Aggertt at haggertt@woodford-county.org or mailed to Woodford County Health Department, 1831 S Main St. Eureka, IL 61530 no later than Friday, March 15, 2019.
